The disease course of children with nephrotic syndrome (NS) who develop a renal allograft (Rtx) after being on immunosuppressive drugs (ISs) for years or even decades is not well known, with the exception of those with recurrent disease. In addition, a late onset of proteinuria after Rtx has not been reported. We conducted this retrospective study to describe the clinical course of children with NS after Rtx and to determine the prognosis of proteinuria after Rtx. The study included a total of 74 children who had received Rtx (group 1) and 74 children with primary nephrotic syndrome who had received a transplant (group 2). Their clinical data, including age at Rtx, age at onset of NS, interval between onset of NS and Rtx, time from Rtx to onset of NS, IS use during follow-up, and disease recurrence of NS after Rtx, were analyzed. In groups 1 and 2, respectively, 49 and 39 cases were newly diagnosed with NS, and the median ages were 11.1 and 7.5 years. In group 1, 11 cases had a recurrent disease. Fifteen cases (20.3%) developed proteinuria after Rtx. Three of these cases continued to have proteinuria without an identifiable cause. All three had been using ISs for 10 to 12 months before proteinuria developed. All three developed ESRD in 6 months, 2.5 years, and 4 years, respectively, after Rtx. In group 2, 32 cases developed proteinuria before Rtx. Three of these developed ESRD in 3 to 10 years, but none developed ESRD in 7 years after Rtx.
